Clinical Images and Privacy
All patients on this website have given explicit consent for their images to be used. The outcome of surgery can vary considerably, and there is a spectrum of anatomical variants that mean no two patients are directly comparable. These images cannot be copied without our written consent, and all patient images can be removed if requested by that person.
Terms and Conditions of treatment
- Surgery and treatments carried out at the Beverley Hospital are requests for treatment made by the patient.
- Patients will have received the patient information document together with the consultation process and will confirm they have read the document and that they understand the risks (physical, mental and financial) associated with the procedure. Patients will have opportunity to seek clarification when needed. The procedure will be discussed with the patient and from the options the procedure selected is most likely to address my presenting concern(s).
- You must recognise that, during the course of the procedure, unforeseen events may necessitate different procedures or additional procedures being undertaken to the one requested. You authorise the medical professionals, medical or nursing, to administer any procedures deemed necessary, in excess of the procedure outlined, to protect your well-being or to limit harm that may occur to you. The authority granted under this clause shall include all conditions that require treatment.
- You understand that all forms of anaesthesia involve risk and possible complication, injury and sometimes, though extremely rare (and not seen or experienced by our team), death. You understand the mode of anaesthesia we have discussed and rationale for that.
- You understand that no guarantee has been given in regard to the outcome or final appearance of the requested procedure. You confirm that I do not expect a guaranteed outcome of final appearance.
- You will be asked for consent to photography and can withdraw any consent given at any time.
- You consent to the medical disposal of any tissues removed from my person as part of the procedure.
- For the purposes of medical education, you consent to the presence of assistants and observers during my procedure as deemed necessary or appropriate by Beverley Healthcare limited. We are recognised experts nationally and internationally and often have observers and assistants.
- You confirm that you understand, and accept, that the inherent risks of my requested procedure may, in unforeseen circumstances cause delays or changes that affect my willingness and ability to return to work. This may result in lost income or mean you incur greater cost than originally expected and were quoted. You accept that Beverley Healthcare limited is not responsible for any extra or unexpected financial burden that you incur as a consequence of undergoing this procedure.
- You confirm that Beverley Health limited has explained to you, to your full satisfaction, the procedure in a manner that you understand. you understand there are alternatives to my requested procedure, which may involve less risk. You understand there are risks to my requested procedure (physical, mental and financial), some of which are described in this document and some of which may be unexpected or unpredictable and therefore currently unknown to you or us. Should unexpected events or complications occur Beverley Healthcare limited will try to minimise the consequences, but it is my responsibility to comply with the advice given. Failure to do so may result in worsening of the complication or unexpected event.
You confirm you acceptance of all statements above (Items 1-10) and risks described in this patient information document. You are satisfied with the explanations given to you by the team and with whom you have had contact.
